Mrs. smith is an 84-year-old resident of a long-term-care facility. she has a history of heart failure and is in chronic atrial fibrillation. her medications include daily coumadin. she is normally alert and able to accomplish most of her activities of daily living on her own. however, this morning mrs smith is still in bed when the aid comes in to check on her. mrs smith is leaning on the right side and drooling from the right corner of her mouth. she is unable to respond verbally, and the right side of her body is paralyzed.
