Write an equation for a sine curve that has the given amplitude and period, and which passes through the given point.

amplitude 5, period 6, point (2, 0)

That is an awfully basic assignment. You would have to understand the answer before you could understand the answer. In other words you simply have not learned the first lesson in the course. Catch up on your studies! 

Specifically: 

The equation for a sine wave is y = sin(2πx) and that wave has an amplitude of 1. For an amplitude of 1/4 the equation would be y = (1/4)sin(2πx). If you want the equation to pass through a given point (j, k), you subtract the coordinates: y - k = (1/4)sin(2π(x - j)).
