Ms. Clarke is 23 years old, gravida 2 para 0 at 26 weeks gestation. She is admitted to the maternity unit for abdominal pain and elevated blood pressure. She denies leakage of fluid and vaginal bleeding. Ms. Clarke is very upset about her 40lbs weight gain. She relates to the nurse that since she became pregnant she has lost interest in many of her physical and social activities. Ms. Clarke states she is bored, depressed, and very unhappy about her appearance. She says her eating habits have changed considerably. She snacks while watching TV and rarely prepares a meal and she currently smokes a pack of cigarettes per day. Past medical history:

Gestational diabetes, chronic hypertension, stillbirth at 28 weeks gestation. She is currently not taking any medication or prenatal vitamins and has no prenatal care since her first ultrasound at six weeks.
