The only doctor, whose study notes left much to be desired, and obviously could never be wrong in their diagnosis, was the only hope we had left to know what had happened to my daughter to faint.

Here the dramatic irony is observed when referring to the low grades with which Doctor Gaynor obtained his diploma, questioning his ability as a doctor. To make matters worse, being the only doctor in the town, he was the only person who could attend Eileen, with which the situation of insecurity that Bill Carmody feels is ironically dramatic.
