A fencer has a grade 2 ankle sprain. After spending an hour in the athletic training clinic applying ice, compression, and elevation, the athletic trainer decides that the athlete should be sent home on crutches. The athlete indicates some reluctance to use the crutches because he has never used them before. What instructions should the athletic trainer give the athlete so that he can correctly and safely ambulate on crutches?

Instructions on Standing, Sitting and Walking with the crutches should be given to the athlete by the athletic trainer.

Explanation:

To stand up, both crutches should be held by the hand grips in one hand, then,  pushing up with the other hand on the chair will move the weight from the chair to the crutches. Each crutch should then be positioned under each arm. After assuming this position, the arms should be left to hang down over the crutches, maintaining a 2 inch space between the armpit and the top of the crutch. The elbows should be bent to about 30 degrees. To ensure the hand grips are in proper position, the hand grip should be at the level of the wrist  when holding it.  

To sit down, both crutches should be placed in one hand by the hand grips and the other hand should be used to reach for the chair to transfer weight to the chair.

To Walk, weight should be transferred through the hands to the crutches by squeezing the crutches between the upper arms and ribs.

Gradually moving the crutches forward, followed by moving the injured leg forward first, then the stronger leg, produces movement. Majority of the weight however should be put on the crutches through the arms and hands as it is advisable to put only the limited amount of weight possible on the injured leg.
