Why is a health care workers personal appearance important?

Because it helps the relationship between the worker and the patient.

Would you feel safer with a muscular man, long hair, tattoos, and piercings. Or an elderly women with gray hair pulled back in a bun with a warm smile.

Iwould describe it as an overall negative image of themselves.  because they are anorexic, they must sometimes look at themselves as overweight or even obese. however, other people may comment on the being to skinny, which may cause them to see themselves as too skinny at other times.
