Project: Constructing a Database Assignment Introduction
For your last project, you fabricated a detailed log of four fictional customer service interactions. Using that paper as a base, construct a flat-field database of the log information from those interactions.

Assignment Directions:
Use a spreadsheet application to construct your database. If you are not familiar with spreadsheet applications or do not have one available to you, you may construct your database in the form of a table.
Your database must contain the following field labels in this order: Time In, Last Name, First Name, Patient Name, Relationship to Patient, Issue, Resolution, Time Out, Follow-up, and Comment.
Your database entries should contain details appropriate to the situations you reported in your prior project and to the level of information a customer service department would need to collect. Example:
