Patient Z has the following data documented. Z is an elderly man aged 70 years; he has been a widower for the last 8 years. Client Z uses a walking stick as a result of bent backbone; he has also been staying alone and cooking for himself. His three children are all grown up with their own families living far from him. He lost his molars and premolars which were replaced with loose fitting dentures. He suffers from hypertension and diabetes; this has resulted to monthly clinics for medical monitoring. Due to his health condition, he is on daily medication and modified diet. During the first nutrition assessment he reported that he had been skipping meals as a result of poor appetite, he had also lost about 10kgs in a span of four months. His weight is currently 50 kg, has a waist circumference of 80cm and 170cm tall. His fasting blood sugar levels were 10 mmol/L, blood pressure of 140/95, he has pitting edema which was confirmed with laboratory tests.
